Below you will find a student enrollment form for driver education through Mansfield Driving Academy.
Your signed enrollment form is a contract with Mansfield Driving Academy. Please read it carefully and completely.
Select the form for the appropriate month you would like to enroll. Remember that enrollment is on a first-come-first-served basis, and only payment in full will secure your spot in a class.
Please check your schedule before enrolling. Students who miss more than five classes can be withdrawn from the course.

Enrollment Forms by Month
Subject to availability, some months offer classes from 4:00-6:00 pm and some months offer classes from 6:00-8:00 pm. Summer classes are are held during the day and in the evening. All classes fill on a first-come-first-served basis. Please enroll early to guarantee your spot. A minimum of 10 students must enroll in a class or it will not make.

*Classes with a holiday may shift days that week to skip the holiday **Accelerated classes may meet Mon-Fri or Mon-Sat to complete the 16 days in less than 4 weeks. Classes must have a minimum of 10 students to make. |
